Subsidy for Kagoshima University students for PCR testing (administrative testing) for new coronavirus infection

Outline

The University will subsidize the amount equivalent to the co-payment of insurance treatment for PCR testing (administrative testing) for new coronavirus infections based on the Law Concerning Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases (Law No. 114 of 1998). However, the subsidy is available for tests conducted on or after November 17, 2020.

 

Source of support fund

Kagoshima University Academic Support Funds will cover this program.

 

Students eligible for support

Students  (including non-degree students)  who have undergone a PCR test (administrative inspection) on or after November 17, 2020, and who meet either of the following requirements

(1) Persons who have been examined at a medical institution under the direction of the Kagoshima City Public Health Center as a person in intensive contact or a person suspected of having being in intensive contact with new coronavirus.
(2) Others approved by the President

 

Amount of subsidy

The maximum amount shall be 2,500 yen per application, which is equivalent to the insurance co-payment incurred for PCR testing (administrative testing). There is no limit to the number of times the application.

 

Number of support students

The number of students to be supported shall be within the financial resources.

 

How to apply

Applications must be submitted online via the university's website.

Applicants must apply within 60 days after the PCR inspection (administrative inspection) according to the following procedure.
The deadline for applications  conducted between November 17 to January 19 shall be March 20, 2021.

(1) Prepare the following four attachments (1), (2), (3), and (4) as image data.

①Account information (signed with your seal)
You can get the documents on the website. ( Excel )  /  ( PDF )
Fill in all necessary items by either handwriting or typing.
Document shall be seal.

②A copy of the bankbook or, in the case of an Internet bank (including accounts that do not issue paper bankbooks), a copy of the screen showing the following items
Page with your name, name of the Financial Institution, Type of Account, Account Number
In the case of an Internet bank (including accounts that do not issue paper bankbooks)  a copy of the screen showing the account information such as the "account number.

③Receipt for medical examination expenses
If possible

④Medical Statement 
If possible

 

The size of the uploaded images or files should be under 5MB.
If larger than this, please lower the resolution, keep the original document after submission.

(2) Go to the following URL and enter the necessary information, upload the images of the attached documents (1), (2), (3), and (4)  and send the form.

Return of the subsidy

If any of the following is found to be the case, the student may be required to return the entire amount.

(1) When it becomes clear that the applicant has made a false application.
(2) When it is confirmed that there is any other violation of the purpose of the subsidy.
